SERVICE AVAILABILITY NOTICE & PAYMENT INFORMATION
Clinical Mental Health Counseling:
Available only to Oregon residents, in-person or via telehealth.
Mental Performance Coaching:
Available nationwide via telehealth. Your location determines which services you’re eligible to receive. We’ll clarify this during your free consultation.
Individual Session Rates:
$185 per 50-minute session
Services include:
- Clinical mental health counseling (Oregon residents only)
- Mental performance coaching (available nationwide)
- Performance improvement and competition preparation
- Injury recovery mental support
- Assessment and treatment planning
Group Services Rates:
$50 per person, per hour
Minimum: 6 participants, 2-hour session
Group Services include:
- Team dynamics workshops
- Parent education and support groups
- Coach development training
- Mental skills training groups
Insurance Coverage (Oregon Residents Only):
For Oregon-based clients receiving clinical mental health counseling, I accept the following insurance plans:
- Care Oregon
- Health Share of Oregon
- Jackson Care Connect
- Columbia Pacific CCO
- Aetna
- Providence
Important:
Insurance only covers clinical mental health counseling services (therapy with diagnosis). Mental performance coaching is not insurance-billable and is private pay only.
Out-of-Network Insurance:
If your insurance plan is not listed above, I can provide detailed superbills for you to submit for out-of-network reimbursement. Reimbursement rates vary by plan.
Out-of-State Clients:
I cannot bill insurance for services provided to residents of other states. Mental performance coaching (non-clinical) is available nationwide via telehealth on a private pay basis.

When Payment is Due:
Payment is due at the time of service.
Sliding Scale:
Limited sliding scale slots available for clients with financial hardship. Please inquire during consultation to discuss eligibility and available rates.
Cancellation Policy:
24-hour advance notice is required for cancellations or rescheduling. Late cancellations (less than 24 hours’ notice) or no-shows will be charged the full session fee. To cancel or reschedule, please contact me by phone or email.
Exceptions:
Cancellation fees may be waived for documented emergencies, serious illness, or other extenuating circumstances at my discretion.
Good Faith Estimate:
Under federal law, you have the right to receive a Good Faith Estimate of expected charges before receiving services. You will receive this estimate during your initial consultation.
